It be purely cosmetic, I don't think punishing players who take more damage in fights even more than just, leaving with less health is very fun game design. But I think it'd just be cool to see

If this already exists someone please point me to games it is in.

Or a brutal slash from a blade leaving a huge cut across your chest, or massive scratch in the armor you have equipped.
A blunt weapon leaving dents in a shield or plate armor.

I just like the idea of the visual development of a character that comes with a detail like that.
