Some interesting insights here, including that people are much more influenced by *people they follow* switching platforms than by their own *followers* switching platforms.

YES! I was persuaded we would mourn Henry‘s death at the end. Using past tense to talk about him really threw me off.
I also enjoyed the scientific literature device of a case report to illustrate a disease turned a novel very much. #NephJC

P-values are fraught with hazard, but SO IS BAYESIAN STATISTICS
the Achilles heel of Bayesian stats is that someone determines a pre-test probability distribution (usually arbitrarily)
The pre-test probability is often too high
This inflates the final posterior probability

A little goes a long way, and just by bringing up advanced care planning and asking a few questions about their preference even without a specific framework you‘ll probably deliver more EOL care than many other centers

GFR at RRT initiation trend from 2002 to 2022. GFR and peaked in 2010
The trend in the US was to start at higher GFRs throughout the 2000s and this peaked in 2010 and since then there has been a small decline in mean GFR at initiation but imo it is still too high.
Visual abstract - still needs work since KI doesn’t get professionals to do it! 🤷🏽♂️
Interesting data from Alberta

GFR < 15 is classified as ‘kidney failure’ in most studies - but about a third of them die before starting RRT.

Summary: urinary anion gap is not a good measure of urine ammonium and should be abandoned.
If you need to know the urinary ammonium, measure the urinary ammonium.

A graph of the least-squares mean ratio of the change from baseline in the urinary albumin-to-creatinine ratio in the full analysis population.
In the CONFIDENCE trial in persons with chronic kidney disease and type 2 diabetes, combination therapy with finerenone and empagliflozin led to a greater reduction in the urinary albumin-to-creatinine ratio than either drug alone. Full trial results: nej.md/4jEVbbk
