#NephJC in AKIKI-2 BUN of 140 seemed too late/associated with harm, so there is some signal
#NephJC Yes and median (IQR) serum creatinine was 4.4 (2.9-6.7) mg/dL (despite receiving dialysis up until then)
#NephJC The traditional approach is increase in native renal function and then dialysis cessation. We proposes a different paradigm: dialysis cessation to promote increase in native renal function.
#NephJC it is only after 14 days that we know GFR has recovered enough that we go back and identity ‘1st day of recovery.’ We think some have misunderstood the curves to think we were providing unnecessary dialysis in the control arm at time of randomization.
#NephJC it is important to point out that ‘1st day of recovery’ was defined retrospective. On that day, GFR may be very low (so not ‘recovered’ in the way many people use the word). See 2 figures below for illustration. Both these patients contributed to those Kaplan-Meier curves separating early.
#NephJC those are based on the AKIKI ‘late start’ triggers, we basically took the same triggers and said not to continue HD unless we hit the same triggers, it was safe in AKIKI
