They should be on the mayu models by default when you import them into unity.
I have only used the models from versions 3.5.5 and 3.5.7 in my testing

The emulator should be fine.
It looks like you are missing the UVDiscard UVMap on the model. The body disappearing with that single checkbox is what makes me think that.

Also my video example showed the Oyama version.
Your model appears to be the Tora version, so make sure you are using the matching prefabs for it.
It looks like you selected the right ones so I'm not sure why the ears are moving higher.
I tested with the 3.5.5 and 3.5.7 version of the Mayu.

It requires the UVDiscard UVmap on the base Mayu model which is UV2. on the body map it uses the corner ones to remove the fingers and toes, and on the head map it uses the ones marked to hide the head and ears (the tora only needs the two bottom ones, the oyama needs the other 3)

Here is a quick video of me setting it up from unity. This assumes you have everything needed for the Mayu already. Azuki has tutorials for getting the Mayu setup if you need instructions on that. This uses the toggles prefab for the example in vrchat. Hope the video quality remains good.
