High-dose HDF results in increased QALYs at higher costs compared to HD, mainly due to increased dialysis costs. Yet, probability of HDF being cost-effective, across different thresholds, is very high.

In a meta-analysis, online HDF reduces all-cause mortality in dialysis patients compared to HD; the risk reduction appears to be dose-dependent.
Advantage of this IPD meta-analysis is that we overcome certain limitations of individual studies, e.g. lack of power, no randomisation to high-dose HDF, and no power for subgroup analyses.
Yet, methodological and clinical heterogeneity is always an issue in meta-analysing studies.
Thanks @hswapnil.bsky.social and @nephroseeker.bsky.social for discussing our paper.
Obviously also curious on the results of H4RT, will add a lot of new information to the puzzle.
