what you’re describing about local variation is absolutely possible – we already do it for “online consultation” tools. it kind of works but also introduces a fair amount of complication when stuff overlaps or when people have access to services from multiple jurisdictions.
the paucity of possible emoji reactions is really winding me up. the heart is for the compliment, not your story, which unfortunately is all too common and not a thing i like!

i work with a bunch of clinicians. they're doctors and nurses and pharmacists. i am pretty well convinced that, even though we have different job titles, we do the same thing. i am convinced we all do user-centred design.
