I think we need to be decomposing causal effects along these different pathways (test classification, strategy, information, and resource reallocation), in order to know if the costs of the test are necessary or if we could get the benefits along other pathways in other ways. But how to do that --

Thank you! Yeah nah, one can't. The assumption is conditional independence given measured time-varying covariates, handled via inverse probability weighting. Braitmaier et al are explicit that unmeasured time-varying confounders would violate this. Untestable but weaker than full independence.
