Function 🔙 Throw Back Thursday: Eaton at al. demonstrated that kidney-specific knockout of #Dmxl1 disrupted V1–V0 assembly, reduced proton pumping, and raised urine pH, supporting it as a mammalian #V-ATPase assembly factor. Read the full article doi.org/10.1093/func...

Saline Doesn’t Ruin Hyponatremia Labs (and it might help)
How many times have you been evaluating a patient in the ED and bemoaned the sneaky liter of saline they got before anyone sent some urine for analysis. Turns out it doesn't ruin the labs at all.

Potential confounders in studies identifying associations between plasma sodium correction rates and mortality. Na = sodium, AVP = arginine vasopressin A) A perceived causal relationship between an exposure and an outcome can be distorted by a third variable, known as a confounder. B) The serum sodium correction rate is the exposure, mortality is the outcome, and certain comorbidities and their severity act as confounders influencing both variables. C) Advanced stages cirrhosis, heart failure, and cancer are linked to higher mortality risk, sustained AVP release, and reduced kidney function, leading to impaired water excretion and slow serum sodium correction. D) In early stages of these conditions—or in their absence—patients exhibit better survival and, because hyponatremia is usually the result of reversible conditions with relatively preserved kidney function, serum sodium can rise rapidly through large-volume water diuresis.

The fundamental difference between water and salt water (volume) was better known at the time of Homer Smith, where they wondered how the salmon can start and end life in fresh water while spending most of life in salt water
I'm super proud of this work detailing the results of genetic testing in hundreds of kidney stone formers. We identified genetic causes for stones in 34% of those tested (47% if < 16 y.o.). So thankful for such great collaborators & patients! journals.lww.com/cjasn/fullte... #nephrology #mayoclinic
