Guidelines recommend:
1. regular med reviews for older adults taking any number of meds, and
2. government funding to encourage & support this process in primary care.

MedSafer: A New Tool to Support Safer Deprescribing This short video introduces MedSafer, a free electronic evidence-based tool developed by clinician researchers from the Research Institute of the McGill University Health Centre. MedSafer is a computer application that helps guide older adults (60 years and above), caregivers and healthcare providers through the deprescribing process. Patients and caregivers can enter information on their medications and medical conditions to create personalized deprescribing reports on MedSafer. Generated using guidelines for safer prescribing in older adults, these reports can be easily accessed by healthcare providers, helping make the deprescribing process easier and more effective. Canada is trying to cut overuse of antipsychotics in long-term care homes. But the rate is still high | CBC News Use of sedating medications in long term care homes has continued to grow since the pandemic and Canadians with dementia are at a great risk, a new report suggests.

💡 New data from CIHI shows inappropriate antipsychotic use in long-term care remains high: nearly 1 in 4 residents in Canada receive them without a psychosis diagnosis. Reducing to the 15% national target will take time and greater support for LTC staff.
