Valid point - but the same applies to non-statutory inquiries (Kirkup, Ockenden). Their value isn’t just learning; it’s also about answers and accountability 🤔

We have rightly moved away from a culture of individual blame in patient safety. But in doing so, have we created a dangerous blind spot around individual responsibility and accountability?
I’m aware this may be a controversial piece!

Lovely to see you yesterday Lizzi - thanks so much for all your interest and work to push for safer healthcare nationally and locally too.
🚨 INVESTIGATION: Britain's maternity crisis has deeper roots, extending into the universities trusted to train the next generation of midwives. A majority are still promoting a normal birth ideology while women are more complex than ever:

Finally back home after 35 hours of travel - and the weather here is just like it was in Vietnam 🤣 🌧️ 🇻🇳
Keeping a little travel blog for the next 16 days - as a travel from the South to the North of Vietnam to remember Joshua and raise funds for Baby Lifeline. 🛵🇻🇳

Up early for a day of delivering PSIRF engagement training in Lancaster with Morecambe Bay teams…
Operation Anzu has seen hundreds of families sent letters by the NHS asking them to contact Staffordshire police, after a trawl of thousands of records for patients seen by this doctor:
This is so fascinating Helen, can’t wait to see the full programme when it comes out :-)
Agree, this is really significant and hugely welcome step forward.
One of the best things in the Hillsborough Law Bill is not the duty of candour on public officials, as welcome as that is, but news that bereaved families will have publicly funded legal representation at inquests. Finally an equality of arms that's been lacking for so long
A lovely time at the Lantern Festival in Ulverston with my daughter this evening - and the rain didn’t spoil the fireworks at all 😊
A wet and grey day in Cumbria - daughter is back from Uni for the weekend so cooking a roast and lighting a fire :-)
Enjoyed speaking with Liz and Darren from RLDatix at the HSJ Patient Safety Congress this week about maternity safety and the newly announced Amos investigation ⬇️⬇️⬇️⬇️

Good to see East Kent (and Yeovil!) among others on this list of Trusts to be reviewed by Baroness Amos & her team. I'll be discussing maternity care with @dockendenltd.bsky.social, @shaunlintern.bsky.social & @jamestitcombe.bsky.social at the @hsj.co.uk Congress tmw. www.bbc.co.uk/news/article...
A fun day with my daughter yesterday :-)
