Be liberal with the time you think you will need. You could do a week or two, you can also do intermittent if the employer allows it. That’d be like cutting hours or days off during the work week. My employer required me to use all of my sick and annual leave, watch out for that.
I’d see if your doctor is willing to write it up for you with the understanding that you intend to get connected to a therapist. The second you ask for FMLA, you have like 15 days to provide the documentation so I’d be calling the Dr today if you can. Then your employer will respond.
