In short, it takes an understanding of the game you're creating for, the audience of the game, and a level of understanding of original design intent to make those happen. Anything you add has to feel like it should belong there from the start, and improve + enhance. That's what Classic+ is to me.
Something more in depth such as regions of areas attached to existing ones, like the entire EQ Serpent's Spine expansion could be considered Classic+ if the artstyle matched with the rest of the game, too. EQ has always had an art identity problem since Luclin. It'd also need to match gameplay, too.
Removing bottlenecks from various quests because your audience is older now is considered Classic+. Even though it's QOL, it's still an addition on top of the game they know.
Design of raid zones in Classic EQ are basically akin to waiting for something to spawn and then getting to it first. Something mechanically simple like instances could be considered classic+.

As someone who made something that gets compared to the Classic+ of EverQuest, I'd say that the best definition of Classic+ is 'more content in the same style developers in that era'.
For EQ, that could be a new zone with similar graphical fidelity, a new race/class, or a system befitting eras.
