Perspectives on technology: All STEPS count – an integrated framework for net zero urological care Objective To present a narrative review of evidence to guide the delivery of high-quality, low-carbon urological care using a structured framework. Methods Academic and policy papers which outline actions focused on decarbonising urological care and surgical care more broadly were identified and reviewed. The ‘STEPS to Low-Carbon Care’ framework (an acronym for low-carbon care across ‘Settings and Treatments, Efficiency, Prevention and System change’) was then used to categorise and present the evidence-based decarbonisation actions, using the National Health Service in England as a case study. Results Across all STEPS framework elements, tangible actions were identified alongside opportunities for future research and innovation. The evidence-based actions that were identified to transition to low-carbon care settings and treatments included tackling known carbon hotspots in operating theatres: anaesthetic gases, consumables and electricity use. Outside the operating theatre, urology pathway transformation through one-stop clinics, day-case surgery, appropriate use of virtual appointments and streamlined pathways demonstrated opportunities to reduce carbon emissions, with potential additional benefits in terms of cost, efficiencies, and patient outcome improvements. Key climate mitigation actions that support keeping people healthy were identified. There was a paucity of evidence demonstrating the implementation of climate change action as part of routine service delivery. Embedding sustainability across organisational processes and ways of working requires actions to upskill, engage and enable the workforce to deliver and to establish clinical leadership. Conclusion This review identified a range of interventions to decarbonise urological care, whilst highlighting a need for further research. Categorising the evidence according to components of the STEPS framework indicated …
How can we deliver high quality urological care while reducing carbon emissions?
Here we explore & discusses evidence based decarbonisation in urology through a whole system lens, with practical actions

Joe Biden was the president of the United States!
Why on earth was he not diagnosed earlier, before the #cancer had spread?
In fact, we should ask this question about any male over 40.
We have the means to do this. And it starts with just a simple blood test called a PSA.

This is exactly what early detection with a simple #PSA blood test aims to prevent - spread to bones.
New draft Australian guidelines out now for public consultation until 25 May.

Come to Australia, Dr G.
Free healthcare for all!
“Just a little prick.”
Full credit to Barnaby Joyce for highlighting the benefit of early detection of #prostate #cancer using the PSA - which is just a simple blood test.

Latest data from PBCG on MRI prostate biopsy: 2-fold variation in probability PI-RADS 4/5 across 13 sites after adjustment for case mix. Risk of GG ≥2 disease for PI-RADS 4/5 varied between 23%-68% / 49%-87%. Need better standardization www.sciencedirect.com/science/arti...
Ahh, the refreshing sounds of….
adults.
This is something many religious people often misunderstand about atheists.
Sing it, Neil.
It would be very healthy for human species, If there is less Discouragement.

What better cause is there than supporting mental health. As I asked one of my daughters on the weekend: Can you think of anything more important in your life than your mental health?
No, I can't either. Please support us!
"Expanding eligibility for AS and the uptake of AS in men with favourable GG2 PCa is crucial to address the stage shift resulting from the increased accuracy of MRI." bjui-journals.onlinelibrary.wiley.com/doi/10.1111/...
In the last interview of his life (1996), astronomer Carl Sagan gave an eerily prescient warning of the dangers that come when you cannot ask skeptical scientific questions of those in authority. Watch and ask yourself: was he right?
Published an op-ed for @cnn.com: “Nobel laureate: I owe America my success. Today, its scientific future is in danger.”
A personal reflection on what’s at stake as science funding gets slashed. I’d be grateful if you could amplify both in and beyond the science world.

As democracy cracks and the world order shifts:
“This is going to be great television.”
But what do you really think, Fred?
Confused?
Every accusation is a confession.
Notice this.
Now it starts to all make sense.
Also,
Desecration is a word that comes to mind.
