Yes, absolutely the hardest. I never did opioids, but many of my friends who've kicked heroin said quitting smoking was infinitely more difficult.

When my dad had his first bypass surgery the first thing he asked for as they wheeled him into a regular room from the ICU was a cigarette. He never quit and stroked out on the table during his second bypass 6 years later. I even smoked another 10 years (now been 35 yrs since I quit.) Awful habit.
