With a high degree of certainty, here's what happened:
• Car is hard coded to not enter the freeway, they're testing but not ready yet
• Car gets forcibly rerouted onto the onramp by traffic
Oh noooo the car decided to safely stop instead of potentially hurting someone.

At least this one we know was caused by failed pullover, I'm not sure what the other ones were though
