The worrying thing is people in power are listening to these tech bros and believing them.
They only financially make sense with one consultant supervising 3 AAs. I haven't worked at a hospital that could regularly produce 6 lists a day with only ASA 1/2 Patients and put them in theatres near enough to each other to make that work.
We fought for safety.

I start most people on 0.5 to see what happens. If they stop talking and close their eyes then about 2 is a good starting point. If they don't even notice it's probably 3.5+. I use a decent amount of fentanyl bolus for most cases and run remi at 2-3 as propofol sparing adjunct.
I moved to eleveld 18 months ago. I really like it. I find numbers needed more consistent between patients. Did find they were getting a little light after an hour and a half though. But always use sedline. Try to always use it for remi too.
I've been watching Jonathan through med schl for 3 yrs. I turned over stones bc, like a lot, wonder if the real deal. I asked him to post student ID, he did. His schl acknowledged his existence, but shrugged. He wanted help w/research, he showed up. He graduated, I watched that. #TooLegitToQuit
