Hey! @idiots-pod.bsky.social - in choosing between nitrofurantoin and doxycycline for a cystitis caused by a pan resistant ESBL Kleb Pneumo, what would you prefer without AST for any of them?

Increased morbidity − but equally good quality of life for prematurely born adults – Uppsala University

Fascinating long term follow up of 90s babies born pre term with very low birth weight (<1000grams!) by my wife and her colleagues. #skyped #neonatology #pediatrics

Horrible! Do you not get metallo betalactamase testing (NDM and the like)? I only see OXA and KPC

If I recall correctly from @fallofcivilizations.com podcast, the name Greenland was picked to attract settlers from Iceland, making it the earliest known example of real estate marketing by fancy name.

Checked their feed. It’s clearly not the stuff of public health workers or clinicians, neither former nor current. It’s just a bunch of reposts of other accounts’ posts about disease and the situation at CDC and inappropriate jokes like this one about children dying.

They most likely don’t. These types of accounts have popped up from a lot of different agencies, quickly raking up thousands of followers without any proof that they are who they say. The national park one has been clearly debunked.

Is it like a set of news stories (actual and AI generated) that the respondents categorize as fake or real? As part of your survey?

Very interesting. How did you measure ”susceptibility to misinformation”?

Having used an actual virtual boy, what do you think of the ergonomics with the cardboard case with joycons as handles. It almost seems like it would be more comfortable than the original design…

Thank you. That is a AUC 400-600 not taking the MIC 2 into account, yes? We’ve been doing this as well, but can’t find any published data on it and aiming for 400-600 AUC/MIC2 would mean high exposures with trough levels above 20.

Another question about this most interesting study! Are the MICs available for your isolates, especially the CoNS? Did you include isolates with MIC 2 and if so, where you content with AUC 400 - 600 (not doing AUC/MIC2 calculations?)

Very interesting! We are using PEN-FAST at our hospital and will for sure be keeping 👀 on SULF-FAST.

Great! I had heard you guys mentioning something about this in an old episode but wasn’t aware that it was published. Many thanks!

Oh I didn’t realize that Mr PEN FAST was on bluesky. What about @trubianojason.bsky.social himself? Is there a clinical support tool for oral challenge in suspected cotrimoxazole allergy? I suspect we have some sub optimal PCP prophylaxis due to old allergy varnings

Does anyone in #idsky know of a quick and easy PEN FAST-equivalent for co-trimoxazole? @idiots-pod.bsky.social perhaps?

Any Japanese colleagues on #idsky that can tell med about how you’re using the fixed combination of cloxacillin and ampicillin?

Bioavailability of doxy is quite good so I don’t think it would make a difference in most cases.

You wouldn’t do cefepime or meropenem for type I penicillin allergy?

This well researched Dutch guideline changed our practice and there is an excellent Breakpoints episode on the same subject.
