Clinical Pearl: A review of 67 M. abscessus PD infections showed 6 historical cases treated with clofazimine achieved cure. This is the FIRST reported success for a macrolide-resistant strain! Clofazimine is a viable oral combo-therapy option. 5/5
The Results: After 4 weeks of imipenem-cilastatin, amikacin, azithromycin, and clofazimine, followed by 4 months of continuation therapy, the patient achieved sustained clinical cure! He remains recurrence-free at 14 months follow-up. 4/5
The Case: A 63-year-old PD patient developed a recurrent catheter tunnel infection. Initial treatment (catheter removal, debridement, 6-week antibiotics) failed. Retreatment necessitated extensive surgery + a unique clofazimine-based antibiotic regimen. 3/5

5/6 ✅ Outcome: Ascites volume decreased over time, and the patient's weight and appetite recovered. This highlights that "rapid accumulation" can be a vital early warning sign in high-risk patients.
4/6 🔄 Intervention: Despite the lack of classic imaging signs, the rapid ascites accumulation prompted a transition to Hemodialysis and steroid treatment
3/6 🚩 The Clue: After draining the abdomen and pausing PD for just 1 day, 500 mL of fibrin-rich ascites rapidly re-accumulated. Biomarkers (IL-6 & CA-125) were also markedly elevated. The physiology signaled EPS before the radiology did.
2/6 🏥 The Case: A 69-year-old male on PD for 9 years presented with weight loss & anorexia. CT showed bowel dilatation/thickening but NO calcification or fibrous cocoon. Diagnosis was unclear until a "drainage test" was observed.
