4/6 π Intervention: Despite the lack of classic imaging signs, the rapid ascites accumulation prompted a transition to Hemodialysis and steroid treatment
3/6 π© The Clue: After draining the abdomen and pausing PD for just 1 day, 500 mL of fibrin-rich ascites rapidly re-accumulated. Biomarkers (IL-6 & CA-125) were also markedly elevated. The physiology signaled EPS before the radiology did.
2/6 π₯ The Case: A 69-year-old male on PD for 9 years presented with weight loss & anorexia. CT showed bowel dilatation/thickening but NO calcification or fibrous cocoon. Diagnosis was unclear until a "drainage test" was observed.
