I’m worn down by it and do the best I can with the short breaks I try to get, but this could go on for years, and there’s zero support, and mother would refuse respite even if it was an option

Kind of, but the bigger problem is mother’s health…can’t really get away for anymore than 5 or 6 days which isn’t long enough to go back to Australia - and as she hates her carers and there’s no help at all with shopping, laundry etc, I’m stuck
