“Kidney health is the greatest unmet need in diabetes care” Prof. Peter Schwartz, IDF President
Millions go undiagnosed & untreated, too often leading to CKD. Declarations matter, but action at national & local levels is key
It was a pleasure to welcome
Schwartz at our #UNGA side event yesterday

4) Ileal conduit. Not that common, but I had 2 consults for them so
👉When urine ⬆️ in NH4Cl empties into a section of the ileum, NH4 is reabsorbed through the Na-H antiporter of the ileum. Bile acid malabsorption may also cause diarrhea.
👉In sigmoid diversion, Cl is absorbed in exchange for HCO3.
3) Hyperkalemia of any cause including ⬇️ volume and meds. Technically, this is an acquired hyperkalemic RTA, but it does not 🟰 hypoaldosteronism.
👉HyperK impairs ammoniagenesis in the PT
👉K competes with NH4 for reabsorption in the TAL via NKCC2 ⬇️ medullary NH3 gradient for NH4 excretion.
1) Compensation for a respiratory alkalosis. Patients with cirrhosis may have a chronic respiratory alkalosis. In this abstract from @juancarlosqvelez.bsky.social and others at ASN Kidney week, 60% of people with cirrhosis and a metabolic acidosis had a primary respiratory alkalosis (pH > 7.35).
