They might argue & say that diclox/cephalexin is part of their protocol but I just argued that we’re worried about MSSA here & BID is easier to take & the worst ABX is the one the patient won’t take. Also, they make warming pads to heat up to help unclog the ducts. My wife used those after her 1st

I would’ve told them to prescribe cefadroxil since it’s BID instead of q6h diclox/cephalexin. That’s what I had the midwife prescribe for my wife when it happened. Aint nobody got time for q6h!

Pend fungal ball removal in 2 wks. AMB MIC 1. Orig had sternal OM w/ Scedo in 2022 s/p BOLT. We were denied fosmano last yr when the pt had perinephric collection w/ Scedo. Orig POS MIC 0.25. They were on IV & irrigation AMB-D until their kidneys took a hit & they feel terrible after the infusion

Super niche pathogen for sure. But keep on your radar if your patient is in septic shock after a dog bite. They’re usually pretty ill with this organism. Another pathogen associated with dogs is Staph pseudointermedius. 1st thing to ask them when you see it: what’s the name of your dog?!?

One of my favorite teaching points in ID :) however, the treatment doesn’t change but it’s a fun organism to pronounce on rounds

Has anyone noticed a recent change in the taste and texture of Costco pizza? We went and had some slices today and the cheese had a crumbly texture, very salty, and had an odd flavor. Very off putting. It was not the gooey, voluminous cheesy pizza that we once loved.
