7/ Together, the results highlight that stable cognitive phenotypes are observable from the earliest stages of mood and psychosis spectrum illness, with implications for early identification, prognosis, and personalised intervention strategies.
6/ The severely impaired group showed markers of neurodevelopmental risk and vulnerability to poorer outcomes, whereas the average cognition group appeared more resilient—not just less affected by stress, but better able to compensate for it.
5/ A key strength of the study was its characterisation of cognitive profiles using factors relevant to cognitive risk and resilience trajectories, including premorbid adjustment, brain/cognitive reserve, functional outcomes, and stressful-event exposure.
4/ That is, there was a low likelihood of individuals transitioning between the profiles, even though clinical symptoms improved during over follow-up. Hence, the cognitive profiles do not appear to be reflections of current symptoms but rather reflect stable cognitive traits in their own right.
3/ We wanted to know whether these profiles changed over time, & found that over ~11 months of a sensitive illness period typically characterised by increased psychological distress & risk for relapse & suicidality, the profiles were highly stable over time.
2/ We examined cognitive differences in people early in the course of mood & psychosis spectrum illness (n=666) using data from the PRONIA consortium, & consistently observed 3 cognitive profiles characterised by cognition that was:
• Average,
• Moderately impaired, or
• Severely impaired.
