Not arguing. Clumsily saying that vetting wasn't the issue, but the excuses, "explanations" all try to push the blame that way. Someone made a very conscious decision to override the vetting info - not accident or error
Don't think vetting is pass/ fail. It throws up issues that are then used to assess suitability, risks etc.
The vetting isn't the issue, is it? Was the appointment process followed properly, given the results of the vetting process?

Willing to bet a quick check on Companies House shows that nearly all of these were in financial difficulties for at least the last 3 years. Every time I've checked as a result of a story this has been the case. VAT *might* have been the final straw, but most clearly wouldn't have lasted regardless.
