I absolutely share that and make it part of the decision making discussion while eliciting whether that’s in line with the patients wishes.
If the patient has poor functional status and has a high chance of deteriorating 2/2 difficulties presented post trach peg (poor rehabilitation potential, bed bound status, VAPs, skin breakdown, UTIs, protracted course that would set this state as a new baseline)

Another great reason to make ketamine access ubiquitous (easy to get from Pyxis in ICU/ED), my practice in the ICU is to have levophed infusing at a low dose at the time induction meds are being administered, which isn’t always quickly available for RSI. Ketamine would help a lot for that.
I don’t get it. It was very clear to anyone who looked that the research $SAVA used to develop simufilam was fake. Was the market really expecting a positive trial to emerge from fraud? Shouldn’t this obvious fraud have been “priced in” to $SAVA?
