I'vevp been binging Pilot Debrief and while I realize that most general aviation is safe, the whole industry looks like Swiss cheese. In his Debriefs, the most common factor is somebody (pilot or mechanic ) failing a test and then passing it 2 weeks later. Repeatedly. The rules are way too lenient.
