Yes thats why i like the mechanics in Fate which give you the option of “succeed but at a cost” when you technically fail a roll. So much more interesting and it advances the player’s narrative contribution as they can then decide on or at least contribute to what constitutes the “cost”.

If the mechanics mean a character can swing and randomly miss 5 times in a row no amount of tone is gona make that seem heroic.
