I have to admit that Nancy Leveson's "Engineering a Safer World" has languished a bit on my reading list. I pick it up for a case study here and a chapter there from time to time, but reading it in the age of #VibeCoding, #AI or bust, #Agentic or die, etc. just hits differently.

A2.1) Yes, but it's worse than that in many shops. Flattening and leaning out technical orgs mean fewer mid and senior positions and in some cases fewer advancement opportunities (do -> design -> strategy -> lead) in the first place. #ciochat
