This study explored identity-related income gaps among Canadian physicians. A 4-stage theory was developed to explain this: physician understanding of patient expectations, the nature of the expectations, physician responses to those expectations, and financial implications of those responses.

Service restrictions were found to exacerbate participants’ health issues through increased exposure to violence, infectious diseases, and psychological distress.
