Your town might have a Community Services Agency type thing (what is called in MTV) to support seniors, homeless folks, and those trying not to become homeless.
International rescue committee helps resettle folks in the US, from supplies to legal to job training. They are also preparing to help with the malnutrition crisis in Gaza.
I got some recommendations for supporting trans folks, but haven't sorted through them or done due diligence.
Regional planned parenthoods directly provide healthcare. Donations there can help people on Medicaid who don't have another doctor to go to.
