I got a request from someone I did not know to write a letter of recommendation for promotion for another person I did not know. I was really surprised, but apparently this is not unheard of in academia? #academicmedicine #medsky
When the best #clinicians at your #hospital have zero ability to move up the chain, there is something really wrong with your incentive structure. Unlike this very venerable #physician, I highly doubt the equivalent talented clinicians will dedicate 33+ years to #academicmedicine - they'll leave
I also agree with this article that #academicmedicine is a caste system and the #clinical track are lower caste even though that track does 90%+ of the #patientcare. It's especially exasperating when you see wait times stretching to 6 months to 1 year for certain specialties

I’m feeling increasingly pessimistic about #academia and feel ready to leave #academicmedicine because I have not seen one single university with an attached #hospital that has met the moment. Not a single one.

Basically anything from Division Chair to Department Chair to any level of Dean requires tenure, but it seems like a lot of other places are already seeing the writing on the wall and are adapting to a post- #tenure world in #academicmedicine to survive.
We won’t make it unless we change.
I had an interesting conversation with one of my co- #healthpolicy fellows who is in #academicmedicine. Said at their institution can become an Associate Dean from the #clinical track, and was shocked when I said #tenure is required for any leadership roles at my #hospital. 🤷🏻♀️

I get it, #academicmedicine feels like a scam sometimes. my dudes, I may be writing a bill next week to prevent CMS enrolling #olderadults in #MedicareAdvantage without their consent and be forced into #priorauth
This affects clinical practice far more than Journal of 5 people read in last 20 years
#academicmedicine everywhere is the most reactionary institution in this country, and private practices have been taken over by #privateequity and make you treat #patientcare like an assembly line factory, and I don’t want to completely stop seeing patients
