Another aspect of an #EndOfLifePlan is what is known as an #AdvancedDirective #LivingWill
This is intended to inform family as well as medical personnel about your wishes about such things as life support, (non law enforcement) autopsys & who has a say about it
This section MUST BE NOTARIZED

An #EndOfLifePlan starts with the basic (and not so basic) identifying information
You MUST include things such as full legal name (if you go by another name include it as an AKA or AKA(Preferred) name), Government identity number, birthdate, consider having them notarized
