CAD days are a great way to promote digital-self-management of #MSK and #WomensHealth conditions, helping to move care to the community and supporting the wider government agenda.

Our research is a crucial first step in providing a better picture of what care is covered by public money, but it also highlights the serious challenge government faces in shifting care from #hospitaltohome for people at the end of life.

This whole thread is really helpful for anyone not at the #HospitalToHome event (thanks @hbuckingham.bsky.social !)
This post in particular made me involuntarily shout YEAH on the bus just now
Was #HospitaltoHome a poor choice of wording for the conference / call to action? #HomeNotHospital
We need the patient voice at every stage in this and there are plenty of experts by experience willing to help.

Thank you for writing this really useful thread from #hospitaltohome #nuffieldtrust
Thank you to all the excellent speakers and delegates who took part in our #hospitaltohome policy event this morning - we heard examples of successful care shifts in other countries, and wrapped up with calls for greater visibility and parity for care closer to home.
Full recording coming soon!
Conference panel consisting of four speakers
Thank you to @nuffieldtrust.bsky.social for a fascinating #hospitaltohome conference this morning. Really interesting to take a step back and consider international learnings from across primary and secondary care, and, of course, social care.

The first of three excellent threads from @hbuckingham.bsky.social at the @nuffieldtrust.bsky.social #HospitalToHome event.

All of our final panellists underlined need for a serious shift in how politicians and the public see the value of social care.
“If there’s an issue the government should be burning its political capital on, it should be social care [reform],” added #hospitaltohome event chair Isabel Hardman.
Now @theasrstein.bsky.social sums up:
- we need to make community, primary and social care more visible
- the importance of the highly skilled generalist
- it’s not really about #HospitalToHome but living in local communities with excellent care
We live in communities, not in hospitals. They are important but a tiny part of our lives. Should be focusing on how we strengthen communities. The future is here - needs to be funded, recognised and seen. #HospitalToHome
Closing remarks from @theasrstein.bsky.social now. It's been good to have sensible conversations. Common themes - importance of visibility, need for parity in terms of what matters, value of generalism. #HospitalToHome
Talking social care reform:
- Govts rarely get more powerful over time
- Social care reform may burn political capital (costs a lot and ~50% of public think it’s funded already)
- But it needs to happen & we know what to do
So…waiting=frustrating
#HospitaltoHome @nuffieldtrust.bsky.social
Treasury absolutely needs to see narrative on social care as infrastructure in the same way it has childcare says @natashacurry.bsky.social. @theasrstein.bsky.social suggests political capital can be built from value big hospitals place on functioning social care. #HospitalToHome
Can we ever get a proper debate about the funding needed, asks @leonoramerry.bsky.social? Does the assisted dying debate open it up? Yes, to an extent, thinks Isabel Hardman. But the government has the most political capital it will have now, should be using it. #HospitalToHome
Workforce strategy critical to reform says @natashacurry.bsky.social. Fair Pay strategy good but isolated from wider reform agenda - increases costs without commensurate funding. #HospitalToHome
I’m at a @nuffieldtrust.bsky.social event on the shift from #hospitaltohome, and it’s remarkable to me - a historian - how longstanding these issues are. We’ve been worrying about delayed discharge, eg, since at least the ‘70: (as found by me and @victoriabates.bsky.social doi.org/10.1017/jbr....)
What's the role of social workers in social care reform? Quite distinct from care worker role - again not well understood. Real opportunities in the move to 'neighbourhood health service' and role of keeping people well. #HospitalToHome
Needs to show how social care and NHS are complementary, not one subsidiary to the other. Nadra Ahmed clear the plan has to be produced with the sector, and Melanie Williams that politicians made manifesto commitments which need to be delivered. #HospitalToHome
