The four ghost monsters each have a label with their name next to them. Pinky and Blinky are flying through the middle of the folder going right. Clyde is sweating as he tries to escape Pac-Man's mouth. Inky sits atop the Pac-Man logo up top, smiling and facing the viewer with both arms extended. Oh, yes, all the ghost monsters have arms ending in tan mittens.
There are some dots and energizers in the background of the scene, but no maze walls.
The image on the other side of the folder is identical, except it also included a UPC.

Above the maze is the Pac-Man logo in an arc. The other three sides have a purple border with a line of white dots in it. At the bottom is Pac-Man chasing four blue ghosts.
Pac-Man again has an arm ending in a white glove. He now also has legs, this time ending in a naked, toeless foot. The ghosts again have arms ending in tan mittens.
The back image is again identical with the exception of an added UPC.

The information block is filled out (and redacted). The class is filled out as "Anything".
As a bonus, you also get an ad for "Return of the Jedi" (the blue lightsaber poster) that I pulled out of a magazine and put in the left pocket over 40 years ago.
#MazeMonday I did a binder for Star Wars Sunday yesterday, so today Pac-Man gets folders. I bought both these at the same time c. 1982. Can you tell which I used and which I didn't? I believe there's a third design that I'm not sure why I didn't also get. Maybe Kmart (IIRC) didn't have it? #PacMan
