I'm currently putting together teaching on RRT and I realise we have a habit in medical education of teaching indications for a therapy before explaining the therapy.
That's (usually) the wrong way round. Indications make more sense *after* you've explained the therapy.

Have done 4 (2 IJ, 2 SC) w/ supervision. SC emphysema would be the indication I'd think of (though I don't do major trauma).

I'm kind of glad PHEM is so popular because outside is where bears are. I've never seen a bear on the inside of a hospital. I don't even like it when it's cold in theatre or on ICU. Very happy to do all of my medical practice under a roof.
