Reflecting on a hectic week at work after a week of vacation & then 3 days at our national conference #hapc24. I'm so grateful for our #palliative care team here at KU Health System. Lots of good work accomplished in tough situations. Kind words and support flowing in multiple directions.

Big change in PTSD (medical trauma) in the intervention arm. This can have long term effects. #hapc24

Cut to the chase: Inpatient #palliativecare intervention led to substantial improvements in patients’ QOL, depression and PTSD symptoms, as well as caregiver anxiety during HSCT hospitalization with sustained effects on patients’ PTSD symptoms up to six months post-HSCT. #hapc24 #HemeOnc #MedSky

Three-site RCT with 360 adults undergoing autologous or allogeneic #HSCT and their caregivers. Randomly assigned to an inpatient #palliativecare intervention (n=180) versus usual care (n=180). Participants met with a PC at least 2x weekly during the HSCT hospitalization to address symptoms #hapc24

Hospice utilization and LOS no different. 1 key finding is participants voted with their feet. Utilization was much stronger with the telephonic RN follow-up compared to having to see people in a PC clinic. doesn't mean that this replaces PC clinic, they have roles, just may be different. #hapc24

Outcome of EMPallA: QOL improved by the FACT-G score in both arms, symptom burden improved with no difference in loneliness at 6 months. What is interesting is that there were no differences in treatment arm in outcomes, yet interesting to note that a nurse-led program is quite economical. #hapc24

Outcomes were Quality of Life, Healthcare utilization (claims data), Loneliness (3-item Loneliness scale) and Symptom Burden (ESAS).

A bit worried about multiple imputations for missing data, but I would need to read the article to dig deeper on that.

Conclusion - Electronic nudges to promote recommended communication for critically ill patients at high-risk of death did not reduce LOS, but did improve some end-of-life care outcomes without impacting mortality. #hapc24

ICU docs were asked about expected prognosis and if they offered a care plan focused on comfort in accordance with guidelines. Manuscript is under review, so data not published yet, but it looks promising and practical across busy ICU settings. #hapc24 #hapc #ICU

Many guidelines about improving communication in the ICU.

I really appreciate this slide showing the idea of clinical inertia in the ICU. From Kruser et a 2017 www.ncbi.nlm.nih.gov/pmc/articles...

Dialectical thinking is 2 things that seem opposite can both be true at the same time. There are always multiple ways to see something and to solve a problem. Change is the only constant and can move between acceptance and change.

Writing a certain narrative can push us into single story which is dangerous for resilience.

Feel seen, validated in challenges and motivation to improve.
