For the man, all identifying features are replaced with hair. Closest on the right is a person with a face that has all parts twisted and misplaced to the point where it looks as if it came right out of a picasso painting. They are holding a large cake right in front of the patient's face, above the bed. The candles' flames engulf its entire top and create the impression of an enormous, dangerous fire that could catch on anything else in the room at any moment. In the background there is a large banner. It might have been a cheerful message like "Happy Birthday" but the writing is illegible and feels completely alien. Overall, the scene feels threatening, confusing, and alienating as the disease turns what was meant to be a friendly gesture by loved ones into a bizarre horror trip.
