Monash IVF makes second embryo mix-up at Melbourne clinic Monash IVF has mistakenly transferred the wrong embryo into a patient in the second major incident for the company in about two months.

There is no excuse whatsoever for such "mix-ups". Whatever processes #MonashIVF has to put in place, where EVERY single transfer of an embryo is double checked, triple checked, quadruply checked, must be immediately.

Ethically, it is the worst form of medical negligence.
