Pierce the folf (they / them) rests up in bed with pillows and am arm supporting their head while their stomach – filled with somebody of similar size to them (implied to be Koko, the artist) – noisily works on its meal. The folf looks relaxed, proud, and satisfied, with one paw gently resting against the lump of whomever is inside their guts stewing away.
