I’ve occasionally seen big differences, particularly in post cardiotomy shock and those on high dose vasoactives.
Did a dive for evidence to support it…theres some but not lots. @pulmcrit.bsky.social has a nice review of it here:

But Matt, fluid responsiveness is bad, all people must be at the top of the Starling curve at all times…. #dogma

Sounds like Santa’s lesser known 3rd list!
“For everything”
I’m occasionally guilty of this but becuase we cover both ICU and HDU.
I’ll often document ‘day X critical care’ to cover people who’ve spent time in both units.
Anyone who refers to their hospital/department as ‘their shop’.
@jesskellynt.bsky.social RE the Spotify and Alexa issue, if the user sets up different profiles on the Alexa app for each family member then they can log into each device with their profile and play separate music.
Means they can all under the one account.
Even less of an excuse for researchers and publishers to engage with the other place! #Xodus
surely its easier just to say 'circ'?
This thread from @doced.bsky.social is excellent.
My key takes homes:
- When is and how do we define T0 in sepsis?
- Sepsis syndromes are very heterogenous
- The evidence behind sepsis bundles is imperfect.
In the interim, empiric abx and senior input are the best we have. #sepsis
I think the Medical Council have gotten this one bang on. A shared regulator for doctors and PAs is a bad idea.

Perhaps the most fascinating part of the #LeadersDebate is how disparate the views of who the winners and losers are.
Thankfully the discourse on #IrishSky far more moderate than elsewhere.

Hello from the official Australian and New Zealand College of Anaesthetists (ANZCA) Blue Sky account! We're one of the largest specialist medical colleges in Australia and New Zealand, and the region's foremost authority on #anaesthesia, #painmedicine and #perioperativemedicine.
I might live-post the 10-way Royal Rumble-style debate.
We live for the chaos.
Let the chaos begin! #ge24 #RTEUpfront
Food for thought here.
Would be great to see smaller samples used more frequently in adult medicine.
As an aside, I think we need to hold a mirror up to organisations who continue to utilise a platform which stands at odds with their stated aims, missions and culture.
...more journals, universities and major organisations choose it as their primary platform.
Couldn't agree more!
Yeah, I think I’m just going to keep calling them posts.
I think it’s the only time where I reflexively treat a proven or assumed aspiration event.
I’m not sure why, perhaps for fear of a febrile response.
Strong work!
