Check out this terrific paper by George Chamilos and colleagues on the role of albumin in defense against mucormycosis. It was a treat to write the preview with Katrina Mar.

Brilliant statement

I’ve been very impressed with how @ameracadpeds.bsky.social has been fighting the horrific misinformation and science denialism peddled by RFK Jr & his minions

🎓 Huge congratulations to Kathleen Mills from the @fungalspore.bsky.social Hohl Lab on successfully defending her thesis: "Epithelial-Immune Cell Crosstalk Orchestrates Pulmonary Immunity to Aspergillus fumigatus" Her strong, dedicated presence in the program will truly be missed! 💫🧬

And a real doozy from the article:

"Most botanists from the mid-19th century viewed their study as a research backwater."

We're working hard here to change this long-held stereotype!

A Fungi Pioneer’s Lifelong Work on Exhibit Mushrooms in 19th-century watercolors: The paintings of a self-taught female mycologist are featured at the New York State Museum.

"You are my only friend in the debatable land of fungi”

Wonderful quote from by Mary Elizabeth Banning, a self-taught mycologist who described 23 new species of fungi in Maryland in the 19th century.

GM-CSF–mediated epithelial-immune cell cross-talk orchestrates pulmonary immunity to Aspergillus fumigatus During mold infection, epithelial-derived GM-CSF licenses neutrophil killing of fungal cells, leading to improved survival.

So happy to see this story come together. Check it out! Kathleen Mills and many collaborators - GM-CSF–mediated epithelial-immune cell cross-talk orchestrates pulmonary immunity to Aspergillus fumigatus | Science Immunology www.science.org/doi/10.1126/...

It's reasonable to give 5 mg/kg ambisone + posa up front since posa load takes time. I generally don't use prolonged (>2 weeks) dual therapy for mucormycosis. I don't use 10 mg/kg for mucormycosis due to toxicities. The key here is to get isolate and perform AST to optimize therapy quickly.

If Mucorales are in the picture, I do not think that posa and isavu are equivalent. Posa IMHO is a better drug than isavu. For aspergillosis, posa and vori are my top choices, isavu OK too. I only use isavu when patient cannot tolerate posa or vori.

Acute-angle branching is mot consistent with aspergillosis. Were septations present? I would treat with posaconazole which is enriched in CNS-penetrating neutrophils. For a very, very long time. Would try to culture tissue for isolate for ID.
