👉Before we start, a few reminders! Are you new to #NephJC ? Not sure what to do or how this works? Here is a quick rundown, an infographic made by Sinead Stoneman

Hemoglobin targets have swung widely:
📈 Normalize Hb → CV harm signal
📉 Conservative targets → persistent symptoms
Have we found the sweet spot yet?

Anemia in CKD has shaped nephrology for decades
ESAs, iron, transfusions, now HIF-PHIs
But one question remains:
👉 Are we treating anemia optimally or cautiously?

🩸Blood is thicker than water 🌊
Thankfully we thinned out the 2026 KDIGO Anemia Guidelines for you to peruse.
Come discuss what's new.

Anemia research in CKD has had so much big fraud stories (which we don’t touch in the #NephJC write up) given the $$$
- the Normal Hct shady analysis from Amgen and PIs that @nejm.org allowed and still stays there
- the Roxadustat p-hacking that brought down Fibrogen and tainted HIF-PHIs

Pretest and post-test counseling. (A) The components of pretest counseling. (B) The components of post-test counseling. The goals of the counseling session are depicted in rectangular shapes. Questions that the clinicians should ask themselves are depicted in the white clouds. VUS, variants of uncertain significance.

Totally agree. But given the signal for reduced hypoK and SCD… tempting to think K might be contributing independently #NephJC
Feels unlikely to be pure coincidence. Potassium may be part of the mechanism, not just a bystander #NephJC
In patients with CV disease, the “normal” range may be misleading
Mortality is actually higher at K⁺ 3.5–4.0 mmol/L
while high-normal levels (4.5–5.0) are associated with lower mortality 👀 #NephJC

Glad to have you here! Always great to hear from someone who’s been intentionally targeting higher potassium in practice. Looking forward for some insights #NephJC

🍌🥑Can pushing K⁺ to the high-normal range reduce ICD shocks and hospitalizations?

¿Elevar el K⁺ al rango alto-normal puede reducir las descargas del DAI y las hospitalizaciones?

Last #NephJC we discussed dialysis frequency in AKI-D #TenPostNephJC
We sought to answer an age-old question: Is less is really more? Or less is just simply what it is….less?
pubmed.ncbi.nlm.nih.gov/41201895/
If hemodynamically stable and non-oliguric, then probably no. Ultimately, we treat the patient, not the numbers… using labs to inform, not dictate, decisions #NephJC
#NephJC Popular opinion:
Have you ever “withheld” dialysis because the patient looked good despite questionable labs?
Did you feel brave or nervous? 😅
We restrict fluids, potassium, phosphorus…
But what about restricting dialysis itself?
Let’s talk. #NephJC
