PEN-CONNECT is jointly led by Dr. Kerstin Klipstein-Grobusch and Dr. Ana Mocumbi.
Kerstin specialises in implementation research and scaling up chronic disease care across diverse settings.
Ana leads research on noncommunicable diseases in disadvantaged communities.

📍 The project was recently launched in Maputo, co-hosted by @Universidade Eduardo Mondlane and @NCDI Poverty Network, and welcomed representatives from the EU, the Mozambican Ministry of Health, international research institutions, and local partners.
Over the next five years, we build institutional research capacity and promote cross-country learning between partner institutions in Africa and Europe.
With PEN-CONNECT, we study how to strengthen collaboration between primary care and district hospitals. Our international consortium, coordinated by @UMC Utrecht, includes partners in Mozambique, Germany and the UK.
PEN-CONNECT launches in Maputo
Chronic conditions like hypertension, diabetes and mental health problems are rising in many low- and middle-income countries. For people living with more than one long-term condition, care is often fragmented, with limited coordination between clinics and hospitals.
