Verified ketosis, Mediterranean diet, and unverified carb restriction are three distinct states predicted to produce different immune outcomes in IBD. The middle one — the most studied - may be least informative. Each prediction is falsifiable.

No human study has measured diet, bile acids, and Th17/Treg balance simultaneously in IBD. A 2026 JCC mouse study and a 2022 Nature paper converge on Treg expansion — via BHB, via bile acids. This preprint proposes a cross sectional test of diet on both.
