#PalliativeCare lives where certainty goes to die.
That’s our strength.
It’s also our greatest vulnerability.
When medicine pretends there’s no mystery, people flee.
When we treat mystery as sacred knowledge, we become a sect.
That tension is reshaping serious illness care right now. 🧵

JAMA study: Adding tumor debulking to chemotherapy doesn't significantly improve survival in multiorgan metastatic colorectal cancer (mCRC) patients. 382 patients randomized; chemotherapy+tumor debulking: 30.0 months, chemotherapy alone: 27.5 months.
Among adults with multiorgan metastatic #ColorectalCancer, adding local debulking to chemotherapy did not improve survival and increased serious adverse events.

ASCENT ( Advancing the Science of Palliative Care Across the Lifespan) is a monumental achievement for our field and the future of patients living with serious illness. ascentpalliativecare.org #HAPC #palliativecare #hospice #HAPC26
Research dissemination alone doesn’t change practice. Clinicians must elevate research and apply it. Partner with researchers to shape meaningful questions, open our clinics to research, and encourage patient participation. Progress requires true partnership. #HAPC26 #palliativecare #hospice
#Hospice doesn’t need faster intakes. It needs time.
Time to earn trust, to stabilize families, to prevent the 2 a.m. panic that sends people back to the ED.
Earlier enrollment and stable teams save money because they save people from suffering.

I suppose the answer is to turn to borrowing from traditional institutions. We will have to see the terms of those loans & the consequences they will have on the amount of debt the students will incur, & the limit it will place on them (esp first gen) to be able to generate generational wealth.
Students who have earned their way into their dream institutions may have to forgo that dream. Even with the current borrowing power, many students continue to work through school to pay for expenses, and for students who have families to support, it can be particularly difficult.
This will affect everyone across racial and ethnic lines. Not every exceptional student will be able to receive scholarships to supplement the cost, and not every exceptional student will be able to borrow enough. We can’t count on institutions lowering tuition.
How Will Trump’s Big Bill Affect Your Wallet? www.nytimes.com/interactive/... one consequence of the bill the is the negative effect it will have on upward mobility for students pursuing professional degrees who lack family support or other means to supplement the financial cost of their education.
Should #PalliativeCare be in the business of caring for #CancerSurvivors? It’s more complicated than it seems. We talk with Laura Petrillo and Laura Shoemaker about this topic. #medsky #cancersurvivor
