SCOUT UTI study great. N=768 👍. Interesting observations:
👉 clearly nitro great
👉 pivm also great (held back vs ESBL, but not inferior)
👉 fosfo not great *even with 2 doses*. Were these the non-E coli?
👉 better clinical resolution if micro +ve. Some are not actually UTI?

My super department @warwicklifesci.bsky.social is hiring 6 assistant profs in the wake of recent retirements. We're looking for people in microbiology/infection, cellular disease / immunity; environmental biology; and plant/crop science. #MicroSky warwick-careers.tal.net/vx/lang-en-G...
Everyone who thinks measles is no big deal - starting with RFK jr - needs to read this first hand account of watching a child die of SSPE, a devastating complication that occurs years later.

Planning to see vasa ship in Stockholm. Do we know yet where the one after is going to be?
Yes! Would love journal club. Happy to help if needed, look at tons of papers all the time 😉
Many thanks to @escmid.bsky.social for inviting me!
#ESCMIDGlobal2026 has been fabulous -- it's been such a treat to meet authors, students, friends etc. Thank you so much for all the lovely and constructive feedback about the journal and Lancet in general. My favourite probably was "it was great when you rejected me, the comments were so helpful" 😉

Both @hoferu.bsky.social and I think it’s hilarious that, despite being close neighbours back home, we had to go all the way to Munich to finally catch up in person after…several years I think! #ESCMIDGlobal2026
Late-breaking #Lancet SCOUT RCT results at #ESCMIDGlobal2026:
💊 Day 7 clinical resolution
• Nitrofurantoin: 74%
• Pivmecillinam: 70%
• Fosfomycin (2 doses): 67%
• Fosfomycin (single dose): 59%
🔑 single-dose fosfomycin underperforms vs standard short-course regimens

Pip/tazo vs MER (EI)
✅Non-inferior 7d failure(29.4% vs 33.5%)
❌No early mortality signal
30d mortality 14.4%(interim)

Superb session at #ESCMIDGlobal2026 on the PeterPen and ASTARTE trials
Important results, great discussion, thanks to @angelahuttner.bsky.social and friends
Big remaining evidence gap concerns optimal empiric (rather than follow on) therapy
ACORN study strategy with consent waiver may work
🔑 Take-home from the SCOUT RCT (#Lancet) at #ESCMIDGlobal2026:
Single-dose fosfomycin underperforms
Nitrofurantoin is most effective option for uncomplicated UTI.
More patients on fosfomycin needed additional antibiotics
👉 Time to re-evaluate first-line rx and update guidelines.

Yes, good point. But is that right? If people are using AI as a doctor replacement, I would be much happier if there were proper privacy and safeguards.
Of course people agreed to all of this in the terms of service. That they probably didn't read...
PS Besides privacy, I worry about people with genuine health issues seeking help from AI (only). After reports of poisoning, suicide, psychosis etc. What are the safeguards? What about misdiagnosis and mistakes?
Also, no data or code sharing. I guess that's private then.
Interesting paper, but hard to judge. How much of the queries are medical? Fitness? Nutrition? How well did the scrubbing, transforming, and classifying of queries work?
"Copilot evaluation activities are not designed to generate generalizable knowledge or evaluate clinical hypotheses. As such, these activities were reviewed by Microsoft’s Corporate, External and Legal [...] and determined not to constitute human subjects research". Is this the new ethical approval?

Short piece in The Lancet - best thing I've read this week.
"Performative cruelty & unchecked abuses of power are defining political leadership, fracturing the cooperative values of global health..." then on to explain how such contempt influences behaviour & new norms.
Depressing but insightful.
I love the U-Bahn design in Munich, so trippy. The bright orange at Marienplatz is the best.

Programme overview
Rare selfie of editor in the wild
Good morning #ESCMIDGlobal2026 -- if you're up early come and join me and other editors in ICM 13b for a session on overcoming challenges in publishing.
Eek, good luck! I'm starting to think it was a bad idea to book a flight Thursday late for a talk first thing Friday morning...
"Nearly half (49.6%) of responses were problematic: 30% somewhat problematic and 19.6% highly problematic".
Fluffy tabby cat with a big yawn
Just typed "I wish you the beast of luck with publication elsewhere".
Someone needs a vacation after ESCMID 🙄
