Proposed mechanism of action for remote ischemic preconditioning. Multiple mechanisms for remote ischemic preconditioning have been suggested. A novel mechanism of action shown here involves high-mobility group Box-1 (HMGB-1) and possibly other molecules, released from muscle in response to transient ischemia. Transcriptomic imprinting of renal tubular epithelial cells occurs following signal transduction from engagement of HMGB-1 with Toll-like receptor 4 (TLR-4) on proximal tubular epithelial cells.
