What a really cool concept!
A ton of thoughts! (but no evidence...yet). I think both low and high dose strategies can be considered on a patient by patient basis, in conjunction with Addiction Medicine. It's a frustrating question as this is becoming more prevalent without substantial evidence to guide clinicians.
For transparency - I'm biased regarding this topic (it's my favorite!), however, I really like the following papers:

Unfortunately, there's not much evidence regarding MOUD initiation and/or continuation during critical illness.
