At #CCR25, we saw some trials use a #CompositeOutcome (an endpoint that combines more than one outcome into a single measure) & also saw some of the problems these can throw up. I have discussed these before but thought it would be useful to revisit 1/9

Having major withdrawal from #CCR25β¦ Extraordinary event, extraordinary people, in an extraordinary place! @criticalcarereviews.com
CT perfusion and CT angiography did not meet sensitivity and specificity thresholds of 98% to determine death by neurologic criteria (commonly referred to as brain death), a cohort study showed.

Did CTP and CTA enhance the diagnosis of death by neurologic criteria in critically ill patients with high accuracy. #FOAMed #POCUS #FOAMcc @criticalcarereviews.com #CCR25
Prehospital hypoxia, hypotension, and hypocarbia were significantly associated with increased risks of death and unfavorable discharge outcomes after TBI.

Active life expectancy decreases significantly among older adults with increased hospital admissions for critical illnesses, major nonelective surgeries, and other severe conditions.

Patients with vertebrobasilar artery occlusion who received endovascular thrombectomy beyond 24 hours after symptom onset had a greater increase in the likelihood of good functional status at 3 months.

It's the final day of #CCR25! π©Ί The JAMA Network Open team has gathered a few more articles to enhance your takeaways from the meeting: π π
Preoperative use of GLP-1 receptor agonists was not associated with increased risk of postoperative aspiration pneumonia. ja.ma/4dS3mj3
From JAMA Neurology: CT perfusion and CT angiography showed less than optimal accuracy for confirming death by neurologic criteria.

Editorial: The UK-ROX trial demonstrates that targeting lower oxygen saturation in critically ill patients is safe but does not improve outcomes.

Conservative oxygen therapy with a target SpO2 of 90% did not reduce 90-day all-cause mortality compared to usual oxygen therapy in mechanically ventilated adult patients.

π’ #CCR25 Highlights: We've compiled the JAMA and JAMA Neurology papers presented at this yearβs meeting. Explore the thread! π
Augmenting enteral protein did not improve outcomes for critically ill patients compared with standard protein levels in the intensive care setting. ja.ma/4dV8SkX
INDEX trial: CT perfusion wasn't adequate for evaluating brain death.
huge problem: they're using clinical exam as the gold standard
I think most patients classified as brain dead by CT perfusion but alive by exam were dead... #1/2

The North American ventilation trials have great control arm protocols. I direct my residents to read them. (Again this is not why the interventions in those trials fail.) #CCR25
PROMIZING trial: PAV+ equivalent to PSV for weaning
Although neutral, I love this trial π
shows that PSV is a good weaning mode that optimizes synchrony while providing adequate support (with driving pressure ~12 cm)
π keep PSV in mind as a *maintenance* mode (not just for SBTs)

PROMIZING Trial Results presented at #CCR25 in Belfast. Congrats to Dr Karen Bosma and the PROMIZING team. See the full trial results in @nejm.org
Also in multiple septic patients, muscle/end tissue biopsies actually show adequate oxygenation but low ATP - shock is as much of a cellular state as a perfusion crisis and do we even need to perfuse βshockedβ cells? #emimcc #CCR25
For the vast majority of patients in ITU, weaning isn't a problem. It's always going to be difficult to find a superior weaning strategy in an unselected population. #CCR25

A graph showing the cumulative probability of successful liberation from ventilation and of death before successful liberation.
In the PROMIZING trial, proportional-assist ventilation with load-adjustable gain factors did not differ significantly from pressure-support ventilation with respect to the time to liberation from mechanical ventilation. Full trial results: nej.md/4kC2a67

#PROMIZINGtrial results are in - no difference in time to successful liberation from mechanical ventilation between proportional-assist ventilation and pressure support ventilation
