At #CCR25, we saw some trials use a #CompositeOutcome (an endpoint that combines more than one outcome into a single measure) & also saw some of the problems these can throw up. I have discussed these before but thought it would be useful to revisit 1/9

Brain Death Cannot Be Determined by Imaging, Study Shows Neither CT perfusion nor CT angiography met 98% sensitivity and specificity thresholds

CT perfusion and CT angiography did not meet sensitivity and specificity thresholds of 98% to determine death by neurologic criteria (commonly referred to as brain death), a cohort study showed.

INDEX trial: CT perfusion wasn't adequate for evaluating brain death.

huge problem: they're using clinical exam as the gold standard

I think most patients classified as brain dead by CT perfusion but alive by exam were dead... #1/2

The North American ventilation trials have great control arm protocols. I direct my residents to read them. (Again this is not why the interventions in those trials fail.) #CCR25

PROMIZING trial: PAV+ equivalent to PSV for weaning

Although neutral, I love this trial 😍

shows that PSV is a good weaning mode that optimizes synchrony while providing adequate support (with driving pressure ~12 cm)

πŸ”‘ keep PSV in mind as a *maintenance* mode (not just for SBTs)
