Excellent new review on predicting response to the low FODMAP diet. Symptom severity, breath tests + microbiota all explored - but clinical & psychological factors are the most practical right now. Next step: simple validated tools to personalise treatment.

Immunology in Coeliac Disease.

T cells, APCs, B & epithelial cells drive CeD. The Exact immunologic sequence is unknown. HLA-DQ2 & DQ8 are essential. Without DQ2 and DQ8, CeD is (mostly) ruled out.

Amazing to watch commentators and outlets attribute these findings to UPFs and seed oils when the study doesn't even look at this. The assumption that eating more linoleic acid -> higher tissue arachidonic acid enrichment has fueled so many handwavey hypotheses despite metabolic regulation

The #intestine's dynamics (environment/movement/stiffness/flow) shapes stem cell fitness and function, shown here via PIEZO channels and is reflected in in-vitro models. So, are static 3D cultures sufficient? Could microphysiological systems #MPS #Organ-on-chip offer a better approach here?
